Start HereLet's get started with a few questions
Types of leave I can help with
I can help with continuous or intermittent leave, reduced work schedules, federal FMLA, Washington Paid Family and Medical Leave, and employer medical leave.
Using insurance
I am in network with Premera, UnitedHealthcare, and select Blue Cross Blue Shield plans.
If I am in network with your insurance, you can use your benefits for the clinical evaluation. If I am out of network, I can provide a superbill that you can submit for reimbursement if your plan includes out-of-network benefits. If you're not sure about your coverage, I can help you check your benefits.
Common Questions
With a few brief questions, I can help you determine which type of leave may be most appropriate for your situation.
Yes. FMLA can apply to mental health conditions when they meet the requirements for medical leave. Part of the evaluation is determining how your condition is affecting your ability to work and whether leave is clinically appropriate.
No. Leave can sometimes be taken continuously, intermittently, or through a reduced work schedule. The type of leave that is appropriate depends on your situation and clinical needs.
It depends on the type of leave you're seeking. I can help you determine what paperwork you need and where to get it.
No. The purpose of the evaluation is to determine whether medical leave is appropriate for your situation. Final approval is made separately by your employer, leave administrator, insurer, or applicable leave program. Be cautious of services that guarantee certification or approval before an evaluation has taken place.
